Open Enrollment is Mandatory!

• This means that all PEBB members with coverage active in October must re-enroll for the 2027 plan year within the month of October. Flexible spending accounts must be re-enrolled annually to participate (do not roll over), and opt-outs are required to re-enroll annually in order to continue to receive the opt-out monthly incentive.
• Members who complete Open Enrollment will have the ability to update flex spending through December 11, 2026, and all other benefits through February 28, 2027.

If you do not re-enroll in October:

• You will NOT have a medical plan in 2027. You must re-enroll during Open Enrollment to select your new medical plan for the 2027 plan year.
• You won’t have the flexibility to update enrollments unless you have a qualifying change event.
• PEBB will apply surcharges if not already applicable.
• You will not be able to enroll in flexible spending accounts.

Providence Medical Plans – No Longer Available

• PEBB will instead offer two new medical plans in 2027, Moda PPO and Moda Coordinated Care. Employees who reside between Eugene and Portland do still have access to the same Kaiser medical plans.

NEW Moda PPO & Moda Coordinated Care Medical Plans

• This plan will be offered in Oregon and surrounding areas. Moda Coordinated Care replaces the Providence Choice and Moda Synergy plans.

NEW – Moda PPO Medical Plan

• This plan will be offered in Oregon and across the U.S. Moda PPO replaces the Providence Statewide PPO plan.

When you enroll in a Moda Health medical plan, keep in mind:

• PEBB’s 2027 Moda Health plans will use the Connexus provider network. This network includes Providence providers. This means Providence members who choose a Moda Health plan for 2027 may be able to keep seeing their current providers. Visit Moda Health’s 2027 “Find Care” provider search page to search for in-network doctors in your area. You can also call 844-776-1593.
• Prescription drug coverage is administered separately by Judi Rx. For pharmacy questions, contact Judi Rx at 833-895-9846 (available starting Oct. 1, 2026). More information is available under the Tools & Resources slider below.
• You have access to Garner to search for Top Providers. When you use Garner Top Providers for care, you can get money back for the cost of your visit. This can help you pay for things like deductibles, copays, lab tests, X-rays, and prescriptions ordered by a Garner Top Provider. The amount you can be repaid is based on the plan in which you enroll. Review the Garner online guide to learn more about the Garner program.
• If you enroll in a Moda Health plan for 2027, you’ll receive a new medical ID card in late December. You can use it for both medical services and prescription drugs. Be sure to share this new ID card with your current providers and pharmacies. Members can print paper ID cards to use by logging into the Moda member portal.

Garner  – Use Top Providers and Get Reimbursed for Out-of-Pocket Expenses

• Garner helps connect Moda Health medical plan members with high-quality care. The providers listed in the directory are called “Top Providers.” Garner is especially useful when looking for specialists or a new provider.
• When you choose to visit a Garner-approved provider, you can be reimbursed for the costs of your visit. This includes your deductible, copay, or coinsurance. It also includes other services like labs, prescriptions, and X-rays when they’re ordered by your Garner-approved provider. You may be repaid up to:
  • $1,000 per year if you have individual coverage.
  • $2,000 per year if you cover yourself and family members.
• You’re not required to use Garner. Your Moda Health medical plan network will remain the same even if you don’t use Garner.

Delta Dental Plans (Administered Moda)

• Virtual dentists will be added to the network.
• The way pulp capping, bone grafting, and re-cementing are covered is changing. See your plan summary for details.

Health Care Flexible Spending Accounts

• Academic Year Employees – If you are a 9-month employee and are paid during the academic year, PEBB will want to know how many pay events you have in your primary academic year position. Most 9-month employees have 10 pay events in the academic year.

Revive EAP (Formerly Canopy EAP)

• With the name change to Revive, you’ll see a new logo and a new look and feel on the website. You’ll also hear a new name when you call for assistance.
• There will be no changes to:
  • The services you receive.
  • The phone number you call.
• You’ll also continue to receive care and support from the same team of “everyday assistance program” resources.
