2026 Open Enrollment Information
Post Open Enrollment Information
Employees who completed Open Enrollment in October do have the flexibility to update enrollments:
- All other plan elections can be updated through February 28, 2026, by submitting the PEBB Open Enrollment form (changes are prospective and take effect the first of the following month after submission).
- Flexible Spending Accounts can be updated through December 12, 2025, by submitting the PEBB Open Enrollment Flexible Spending Form.
Form(s) can be digitally submitted by uploading to the HR Secure Inbox for processing.
2026 Providence & Moda Reminders
As we roll into 2026, I wanted to take a moment to summarize information that was shared during our October Open Enrollment process.
Providence Members
Beginning with the 2026 plan year, Providence is partnering with Collective Health. There are no changes to your medical plan coverage or Providence Health’s provider network; however, there are several important updates you should be aware of and act on.
With Collective Health, you will now:
- Use a New Dedicated Member Services Phone Number: 855-284-1368. Call volume is high at the moment, so members should expect longer than normal wait times.
- Access an Upgraded member portal and Mobile App: https://providencehealthplan.collectivehealth.com/
- The app is available through the Collective Health website or your app store (search “Collective Health”).
- If you have not logged in yet, select “Register Now” and enter the following:
- Select Subscriber ID and enter your UPDATED medical ID (see below)
- Date of birth
- Legal last name
- Reference your UPDATED Medical ID Number (also known as “Subscriber ID”)
- Providence will now use your existing PEBB ID number as the medical ID number. These IDs begin with the letter “P” and are available to view in the PEBB member portal.
- If you have not yet received your updated ID card, you can find your new ID number by logging into PEBBenroll.com, which is where you completed your Open Enrollment:
- Click “View my current benefit selections.”
- Your PEBB ID/Providence Member ID number appears in the top right corner under “Benefit #.”
- Dependent IDs are listed in the “Dependent Information” section further down the page.
- Request new Medical ID Cards
- You should have received new ID cards in the US Postal mail by now, which list the updated member ID number (to match your PEBB ID), new group ID number, and new prescription ID information. Please be sure to share your new ID card and updated information with your healthcare providers and pharmacies.
- If you have not received your new ID cards yet, you can request new ID cards and print temporary paper ID cards to use in the meantime by logging into the Collective Health member portal or app.
- Receive New Virtual Care Options with Galileo
- Primary care, urgent care, and behavioral health services are available through Galileo. You’ll see a Galileo tile when logged into the Collective Health member portal.
- Be Required to Fill 90-Day Supplies for Regular Maintenance Medications
- Providence and Moda implemented prescription changes in 2026, requiring 90-day supplies for regular maintenance medications from your local in-network pharmacy or mail-order pharmacy. For more information, please review the 2026 PEBB Open Enrollment Guide.
- Providence members can receive prescription assistance by contacting Collective Health advocates at 855-284-1368. Call volume is high at the moment, so members should expect longer than normal wait times. Additional details can be found in the 2026 Providence Member Resource Guide.
- Providence and Moda implemented prescription changes in 2026, requiring 90-day supplies for regular maintenance medications from your local in-network pharmacy or mail-order pharmacy. For more information, please review the 2026 PEBB Open Enrollment Guide.
- Save on Out-of-Pocket Expenses with Garner Health
- Providence and Moda members now have access to the Garner Health program. Garner connects you to quality doctors who are in-network and can help you save up to $1,000 per member, or up to $2,000 per family, on out-of-pocket costs for care. You’ll see a Garner Health tile when logged into the Moda Member Portal or into the Providence Collective Health member portal.
- You’re not required to use Garner. Your Providence Health or Moda Health medical plan network will remain the same even if you don’t use Garner.
- Moda and Providence members utilizing the Garner program have a dedicated helpline that can assist with questions. The Garner helpline can be reached at 458-488-4828 or by emailing concierge@getgarner.com.

Moda Members
Beginning with the 2026 plan year, Moda is partnering with Aetna PPO for out-of-area services. There are no changes to your medical plan coverage; however, there are several important updates you should be aware of.
- New Aetna PPO Out-of-Service Network
- When you’re outside of Moda Health’s service area, you will have access to an Aetna PPO network. This network is called the Aetna® PPO Network through Aetna Signature Administrators®. You can visit these Aetna PPO providers where Moda Health providers aren’t available. This includes care that’s not urgent or an emergency.
- Request New Medical and/or Dental ID Cards
- You should have received new ID cards in the US Postal mail by now, which list the updated plan information. Please be sure to share your new ID card and updated information with your healthcare providers and pharmacies.

Do I need to Complete Open Enrollment?
YES! Open Enrollment is MANDATORY this year for all members with existing coverage. Flexible Spending enrollments and Opt Out status must be recertified annually in order to be eligible to participate in the following year. PEBB will allow corrections to be made after Open Enrollment, but only for members who completed the re-enrollment in October. All other changes would need to be due to a permitting qualifying midyear change event.
One-Step Process to Complete Open Enrollment
- Go to PEBBEnroll.com to elect 2026 benefits online (open for re-enrollment October 1st through 31st).
- Paper PEBB enrollment forms are also available and must be completed in October and submitted to the HR Secure Inbox:
Those who complete Open Enrollment will have through December 12 to correct Flexible Spending enrollments, and through February 28 to correct all other benefit enrollments.
Please note that the Health Engagement Model (HEM) health questionnaire is no longer required, and the $17.50 monthly incentive for completing the HEM is no longer available (more information listed under the ‘What’s New’ slider).

Your home zip code or your work zip code determines what Core benefit plans are available to you (medical, dental, and vision). All members enrolled in PEBB plans will automatically have $10k of guaranteed issue basic life insurance coverage. Below are the Core benefit plans available in Southern Oregon:

THE STANDARDLIFE, DISABILITY, & TRAVEL |
Standard Insurance Company provides group Life, Accidental Death, and Dismemberment (AD&D), Short Term Disability, and Long Term Disability to PEBB members. The Standard can be reached at 1-800-242-1888 from 8 am – 5 pm daily. ASIFLEXFLEXIBLE SPENDING ACCOUNTS |
The State of Oregon offers pre-tax spending programs you can use to save money. The Health Care Flexible Spending Account is a PEBB-sponsored benefit program that allows you to use pre-tax money to pay for certain out-of-pocket medical, vision, and dental expenses, and now certain over-the-counter items. Enrolling in the Health Care FSA can lead to savings of up to 40% on each dollar that is set aside for medical expenses you are going to incur anyway. The Dependent Care Flexible Spending Account allows you to save on child or elder care expenses (e.g. babysitting, daycare, in-home care for an older dependent incapable of self-care). Flexible spending account participants can file claims via the free mobile app, online by signing into your account, or by fax at 877-879-9038. Please also consider signing up for direct deposit and electronic communications via email or text as this will also reduce paper usage and wait times.
